Oral Appliance Therapy (Mandibular Advancement Device)

Custom-fitted dental device that advances the lower jaw during sleep to keep the airway open, offered for snoring and mild-to-moderate obstructive sleep apnea, usually by dental sleep medicine practices working with sleep clinics.

AASM/AADSM guidelines endorse custom oral appliances for mild-to-moderate OSA and CPAP-intolerant patients, with good evidence for AHI and snoring reduction though somewhat less efficacy than CPAP.

Custom-made medical devices under MDR fitted by licensed dentists; in Spain fitting is restricted to registered dental professionals.
United States
Class II prescription devices fitted by dentists following a physician OSA diagnosis; over-the-counter boil-and-bite versions are not equivalent to custom devices.

Fitted by DHA/DoH-licensed dentists with MOHAP-registered devices, generally after physician diagnosis.
